My Life, My Health is the Chronic Disease Self Management Program developed at the Stanford University Patient Education Research Center http://patienteducation.stanford.edu/training. Six consecutive 2.5 hour workshops are lead by trained peer leaders in community settings. People with chronic conditions and care givers may attend. Workshops are highly interactive, provide mutual support, and build participants’ confidence to manage their health and remain active. Topics include:

• Dealing with difficult emotions
• Physical activity and exercise
• Muscle relaxation and guided imagery
• Pain and fatigue management
• Healthy eating
• Communication skills
• Medication usage
• Making informed treatment decisions
