Tomando Control de Su Salud fue desarrollado por el Centro de Investigación para la Educación de Pacientes de la universidad de Stanford. Este programa se imparte una vez por semana por seis semanas. Cada sesión es de 2.5 horas. Las clases se ofrecen en la comunidad, en centros dedicados al servicio público como centros comunitarios, centros para personas mayores, iglesias, clínicas comunitarias, hospitales etc. Los cursos son facilitados por dos entrenadores que han sido entrenados especialmente. Este programa es para cualquier persona (de 18 años o mayores) que tiene una o más enfermedades crónicas asì como parientes o amigos que cuidan a personas con enfermedades crònicas.
